Test Review Grievance Process Effective 5/1/2026 (Updated)

Outlines the grievance process available to Nurse Aide I candidates for North Carolina

A candidate may request a review of your testing conditions and/or test results.  If requested, this review will evaluate the testing conditions and/or accuracy of your test results.

Filing a Test Review Grievance Process for NNAAP® Examination

  • Candidates may  call, send a chat or email Credentia before submitting a Test Review Request Form and paying a $10 fee.
    • Customer Service will  explain the scoring process and testing conditions and/or test results review process. If a candidate wishes to discuss any specific factor they believe affected their exam score, Credentia Customer Service agent will direct the candidate to complete the Test Review Request Form for any concerns related specifically to their score.

  • All test review grievances must be submitted on the Test Review Request Form through the Credentia Platform.
  • Candidate must provide as much detail as possible on the Test Review Request Form to ensure a thorough evaluation. This includes all relevant details and supporting information.
    • If relevant information is not originally included, it may negatively impact the evaluation.
    • However, candidates may submit additional details through a support ticket up to three (3) business days (excluding holidays), after their original Test Review Request Form submission. However, Credentia strongly encourages candidates to include all information in their initial Test Review Request Form.
  • Candidates are permitted one test review grievance per test date per testing event.

The Test Review Request Form must be submitted within thirty (30) calendar days (excluding holidays), after the examination date. After this thirty (30) day period, no test review grievance submissions or payments will be accepted from a candidate.

Re-evaluation and Outcome

  • Upon receiving the candidates Test Review Request Form, Credentia will conduct an evaluation.
  • Once the evaluation is complete, the candidate will receive an email notification from the Credentia Platform informing the candidate of their outcome.
  • The outcome of the re-evaluation may or may not indicate a change in score or status of the exam (e.g., pass/fail).
  • After a candidate reaches the age of 18, Credentia will only discuss test results or test concerns with the candidate. Credentia will not review test results or any concerns with instructors/training programs, family members, or anyone else.
  • All decisions are final.

Response Timeline

Candidates can expect to receive a response within fourteen (14 business days, (excluding holidays), after the Test Review Request Form is submitted on the Credentia Platform.

If the Test Review Grievance Is Decided in Your Favor

  • If the test review grievance is found to be in the candidate’s favor, then the test will be rescored and the candidate will be allowed to retest at no additional cost, and the $10 fee will be refunded.

•             If the candidate’s exam is rescored to a passing result, the candidate’s score will be updated in the Credentia system, and the candidate will be notified through the Credentia Platform.  The candidate’s results will be marked as “Completed,” and the candidates registry results will be automatically sent to the State.

If the Test Review Grievance Is Decided Not in Your Favor

  • If the test review grievance is found to not be in the candidate’s favor, the candidates test will not be rescored, and the $10 fee will not be refunded.
